A legal disclaimer

At Rooted Willow Relationship Centre, we want our clients to be well-informed about the services they are receiving in order to make a fully informed decision about participating in therapeutic assessment and/or treatment. Please carefully review the important information below regarding your rights, confidentiality and breeches, as well as our cancellation policy. Initial Consultations 

Rooted Willow has the right to choose not to provide therapeutic services/assessment following an initial consultation if the psychologist/therapist deems this to (i) be a conflict of interest and dual relationship, (ii), an inappropriate referral (i.e., presenting concerns are outside the usual practice/scope of the psychologist/therapist), and (iii) there are contraindications present for successful therapeutic participation. 

Treatment of Minors

We require parental consent prior to treating minors. At Rooted Willow, it is our practice to get assent from the participating minor, as well as written informed consent from parents and/or guardians. At the psychologist/therapists’ discretion legal documents may be requested to ensure proper and legal consent has been obtained. Rooted Willow requires parental/legal guardian participation in an initial intake assessment prior to meeting with the participating minor. 

Confidentiality and Breeches 

Your health information is strictly personal and confidential.

 

At Rooted Willow Relationship Centre, we make every reasonable effort to respect your privacy and protect your confidentiality: in our offices, in our storage of your health records, in our interactions with other people, and in our electronic interactions with you. 

 

There are several situations when your confidential information may be released to other individuals or organizations without your consent: 

 

1. If any current abuse of a child or dependent/vulnerable adult is revealed, then your psychologist/therapist is legally and ethically required to report it to the appropriate authorities. 

2. If you threaten to harm yourself or someone else, and your threats are considered serious, then your psychologist/therapist is legally and ethically required to take whatever protective actions are deemed necessary. 

3. If your health records are subpoenaed by a Court of Law, then your psychologist/therapist may be required to release your personal information. 

4. If you have been referred for assessment and/or treatment by another professional or agency (such as a medical doctor, psychiatrist, Court of Law, or insurance company), then they may require information or documentation from your psychologist/therapist. 

5. If your psychologist/therapist engages in clinical consultation with colleagues, then your personal information may be discussed while keeping your identifying information (such as your name) anonymous. 

6. If you believe and share with your psychologist/therapist that you have experienced sexual abuse or sexual misconduct by a health professional, we are regulated to report this to the appropriate regulatory body/college. 

7. For clients under 18 years of age, parents/guardians may have the legal right to examine psychological records and be notified about safety concerns. 

Cancellations

Rooted Willow Relationship Centre has a 24-hour cancellation policy. This reflects the importance and value of the time that you and your psychologist/therapist have scheduled for your healthcare. 

 

You can cancel your appointment by logging in to your online client portal (via JaneApp), or by phoning or emailing your psychologist/therapist (info@rootedwillowyyc.com). There is a fee of $50 per hour for any appointment cancelled with less than 24 hours’ notice. You can also change your appointment to virtual / telehealth service delivery at any time without any cost. 

 

There is a fee of $150 per hour for any appointment missed without notice or cancellation. Unless otherwise arranged with your psychologist/therapist, any fees for cancelled or missed appointments will be automatically charged to your credit card number on file. Your psychologist/therapist may be able to waive these fees in emergency or unusual circumstances. You are responsible to pay the fees for cancelled or missed appointments even if your psychological services are being paid for by an insurance company or other organization. Most organizations will not pay or reimburse these fees, so please ensure that you understand your specific insurance plan and coverage.

Training and Further Education

Rooted Willow practitioners may request audio/video recording of sessions for supervision and clinical quality assurance (i.e., continuing education/certification) purposes. Your written consent will be obtained prior to this taking place. You have a right to decline this offering and withdraw your consent at any time.
